my poor emerald... Options

i got a beautiful plant for my birthday 2 years ago and it is beginning to look very poor. i got it when i lived in texas from a flower shop and now we live in oklahoma. the ends of the leaves are browning and yellowing, it produces less foliage, and i have tried many different things to save it. emerald is in the name somewhere. it is a non flowering, houseplant with shiny medium green leaves almost heart-shaped. smooth to the touch. it grows about 8-10 in tall and was once a very full plant. not anymore. it slowly started to decline. i tried to repot it in a larger pot...new soil, plant fertilizers, etc. what can i do to save it????? do you know what kind it is??? any help would be much appreciated.
