Please save my plant before I kill it!!!! Options

I have no idea what kind of plant this is. It was my grandmothers and it means alot to me but its on its death flower-bed (Haha). I don't know if it was supposed to be an outdoor plant or if it is supposed to be an indoor plant like we have it. It was barely surviving and was really sparse. The pot had about 8 separate but same kind of little plants coming out of it but wasn't doing too good so I changed the soil hoping it would thrive I ended up with only two plants with a few healthy leaves on it. the other little plants are still there but either have really dried up leaves or no leaves so I trimmed them down.

It is a small plant with perfectly oval medium-green verigated leaves. The leaves are about an inch long and the plant likes to hang over the pot.

Any ideas of what this plant is, how I can make it thrive instead of survive and/ or where I can get another one to suppliment my pot or how to make more of them grow.

I looked up your plant in one of my houseplant encyclopaedias and I found one that looks like yours called a "fittonia verschaffeltii argyroneura". It says some common names for it are "painted net leaf" and "nerve plant".

According to my book, it says that the plant grows in the tropical forests of Columbia and Peru where it covers the forest floor. It is a creeping plant. It needs high humidity and the temperature in the house should never drop below 15 degrees C (59 degrees F) in both summer and winter. Water moderately in winter and liberally in summer. Put it in a light spot (possibly near a east or north facing window) but make sure not to put it in direct sunlight. Plant in standard all-purpose houseplant soil. Repot the plant when you see roots extending out of the drainage holes of the pot. It also says to feed the plant monthly with 10-10-10 fertilizer.
